Courtyard Manor of Wixom is a leader in providing high-quality, comprehensive care for individuals over the age of 35 with severe and persistent mental illness. We specialize in treating older adults with multiple medical issues who need assistance with activities of daily living.

A comprehensive Structured Treatment Program in a home-like setting.
We have highly trained staff from a variety of disciplines to help our residents meet their goals. From direct caregivers to nurses, social workers, activity directors, and physicians, each resident receives consistent, compassionate, and comprehensive treatment.
We offer a variety of evidence-based psychosocial rehabilitation groups and activities so all residents have something to choose from that they will enjoy. We have groups, activities, and outings that are scheduled seven days per week. Our program strives to help residents learn new skills with the goal of moving to a more independent setting!
We have nurses and medication technicians that will help with medication administration. Residents can participate in our self-medication program to learn more about their medications, dosages, side effects, and purpose.
Courtyard Manor of Wixom provides 24-hour supervision and support to our residents. We have a delayed door release system for residents who need this level of support. We provide transportation and support into the community as often as residents prefer!

Our programs are based on psychosocial rehabilitation principles that operate from the premise that each individual has the capacity for growth and has the right to realize their hopes and dreams. CYM staff work with each resident to help them align their hopes and dreams with their capabilities. With this as the guiding principle, the residential program is designed to develop skills necessary to maximize each person’s potential.

A Pre-Placement visit can be arranged so the resident and family can visit and take a tour of our facility prior to moving in.
We’re happy to speak with you regarding potential admissions to Courtyard Manor of Wixom. Once we review the referral packet, we will arrange for an in-person assessment to meet the potential resident.
Upon admission, the assessment process helps us determine each resident’s unique needs. Each discipline completes a comprehensive assessment with each resident. The treatment team, along with the resident, guardian, and natural supports, use the assessment results to develop the Treatment Plan.
Within 30 days of admission, the social worker will use the assessment results to work with the resident to develop their Treatment Plan, also known as the Individual Plan of Service (IPOS). The IPOS guides the treatment provided at CYM and is reviewed with the treatment team within 30 days of admission and semi-annually thereafter.
Who We Serve
Courtyard Manor of Wixom specializes in working with residents with severe and persistent mental illness with co-morbid medical problems. While we accept residents over the age of 35, many of our residents are older adults that require extra assistance with mobility and care of their Activities of Daily Living (ADLs), like showering and dressing. We are barrier-free and can accept residents that use walkers and wheelchairs.
